1) Please read the SI2010 FAQ Sheet before completing registration.

2) Please list any previous dance training/experience that you have had.

3) Which session are you registering for?

 Session A: Beginning Hip-hop Intensive (Ages 6-8)
 Session B: Intermediate Hip-hop Intensive (Ages 13-18)
 Session C: Funksters Leadership Program (Open to current Funksters dancers who are graduating Seniors ONLY! No exceptions.)

**Dancers may switch to a different session within the first week only if they find the skill level isn’t a good fit, but this is at the discretion of the SI coordinators.

4) I will mail the following payment along with a WAIVER FORM to the address below. I understand that my registration is not complete until full payment is received.

 Session A or B – $200 payment
 Session C – $125 payment
